Job Description

Overview

CHI LakeWood Health provides long-term care family practice-clinic 15-bed critical-access hospital public health and assisted living services. Nestled in Baudette MN just minutes from Lake of the Woods and Zippel Bay State Park. Enjoy natures playground lake living trails unique shopping and dining! Check out the link for more info on Baudette MN a CNA at CHI LakeWood Health you will help our patients by providing nursing care to our Long Term Care (LTC) residents under the direction of the physician or registered nurse.

Every day you will work with our patients to evaluate their needs and provide physical or psychosocial care. To be successful in this role you must have a genuine compassion for your patients well-being and understand the role you play in helping our our patients feel at ease. You also have the patience to explain the process answer questions and update documentation.

To thrive in this role you will be organized and prioritize accordingly. You will demonstrate the ability to work independently; have strong written and verbal skills with the ability to engage others. Your leadership skills will allow you to handle conflicts resulting in a positive outcome.

  • Contributes to the assessment of the health status of patients.
  • Perform administrative requirements such as completing necessary forms charts reports etc. and submit these as may be required.
  • Communicate patient information with care teams.
  • Assist patients/families with meals or changing bed linens.
  • Reports any changes in a patients condition to the Charge Nurse.
  • Must have ability to work effectively and collaboratively in a diverse environment.
  • Amongst additional responsibilities.

Qualifications

  • Certified Nursing Assistant or willing to obtain certification for the position within six (6) months.
  • Basic Life Support (BLS) issued by the American Heart Association.
  • Must be at least 16 years of age.
  • Excellent verbal and written communication skills needed.
  • Ability to multi-task and prioritize
  • Ability to maintain high level of accuracy when documenting cares.
