Purpose
The Public Health Nurse 3 serving the Scales NeighborhoodNursing (SNN) program assesses health needs delivers comprehensive nursingservices and implements strategies to improve health outcomes for residentswithin a designated City of Milwaukee neighborhood.

Essential Functions
Care Coordination and Documentation
- Provide direct nursing care including health screenings immunizations and chronic disease management support.
- Assist clients in navigating healthcare systems linking them to appropriate services and resources.
- Develop and implement individualized care plans in collaboration with clients and multidisciplinary teams.
- Maintain complete clear and accurate documentation including case notes and treatment care plans.
- Maintain knowledge of national regional state and local research and public health practices.
Community Engagement and Collaboration
- Conduct community health assessments to identify health disparities resources and needs within assigned neighborhoods.
- Build and maintain relationships with community members healthcare provider organizations businesses other governmental agencies and community-based organizations (CBOs).
- Attend community events and meetings to engage with residents and represent the SNN Program.
- Collaborate with Public Health Nurses Public Health Educators and other health professionals to achieve shared goals.
- Participate in community coalitions and advisory groups representing public health nursing.
Scales Neighborhood Nursing (SNN) Program
- Contribute to the design and implementation of neighborhood-specific health interventions aligned with programmatic goals.
- Advocate for policies and programs that address health literacy social determinants of health and reduce health inequities.
- Facilitate access to health information.
- Collaborate with management in the planning and evaluation of the SNN program to measure programmatic effectiveness and to inform decision making.
- Report progress and outcomes to supervisors and stakeholders.
- Provide guidance concerning incidents errors and other patient service issues.

Conditions of Employment
- Work standard hours: 8:00 a.m.-4:45 p.m. Monday through Friday.
- May be required to participate in a 24-hour emergency on-call rotation schedule including weekends.
- Must wear industry related personal protective equipment (PPE) such as gloves facemask or face shield.
- May travel outside the City of Milwaukee occasionally overnight to attend training and conferences.
- Must be able to lift and move up to 25 lbs.
- May be exposed to infectious agents or toxins.
- May be required to respond to an emergency or broad impact event.
- Required pre-employment tests include:
- Physical/lift test
- Audiogram
- Vision
- Medical evaluation for respirator
- Respirator fit test
- Non-DOT drug test
- Required pre-employment vaccines include:
- TB Test; annually throughout employment
- Hep B Vaccine
- Covid 19 Vaccine
- MMR Vaccine
- Varicella Vaccine
Minimum Requirements
- Bachelor of Science Degree in Nursing (BSN) from an accredited nursing program. (Note: students in their final semester may participate in the selection process but may not be hired until the degree has been completed.)
- Current permit or license to practice professional nursing in Wisconsin OR eligibility to receive such permitorlicense. License must be obtained by the time of appointment and maintained throughout employment.
- Two years of professional nursing experience related to community health preventative health services or patient advocacy.
- Valid drivers license and possession of a properly insured vehicle for use on the job is required at time of appointment and throughout employment. Private auto allowance may be paid pursuant to Section 350-183 of the Milwaukee Code.

Desirable Qualifications
- Masters degree in a related field from an accredited college or university.
- Ability to provide interpretation in Spanish Hmong or another language.
- Experience leading supervising or managing health-related staff programs and/or projects.
Knowledge Skills Abilities & Other Characteristics
Technical
- Knowledge of nursing practice and patient health assessment related to community and public health care.
- Knowledge of the information and techniques needed to diagnose and treat injuries and diseases.
- Ability to efficiently use database spreadsheet word processing presentation and case management information systems.
- Knowledge of mathematics to accurately calculate dosages and take various measurements.
- Ability to read and interpret documents such as safety rules and technical materials.
- Ability to adhere to organizational standards policies and anti-racism principles in all professional activities.
Communication and Interpersonal
- Ability to work cooperatively effectively and fairly with coworkers and residents whose background may differ from ones own.
- Ability to build and maintain effective working relationships with a multi-cultural and multi-disciplinary staff the public various City of Milwaukee officials and other agencies.
- Ability to provide services in a culturally sensitive manner.
- Ability to speak effectively to individuals and to groups and teach and train others.
- Written communication skills to accurately document patient information such as case notes treatment and care plans and to produce written reports and job-related correspondence.

Current Salary
The current salary range (Pay Range 2IN) is$78021-$86854annuallyand the resident incentive salary range for City of Milwaukee residentsis$80361-$89460annually.Appointment above the minimum ispossible based upon level of experience and other qualifications and is subjectto approval.
