About the Opportunity

GMHBA is seeking a skilled Care Coordinator with a strong background in cardiac nursing to join our Private Health Insurance team. This role focuses on delivering a remote Cardiac Rehabilitation program designed to improve member health outcomes and reduce claims costs by providing specialised cardiac care through technology-enabled monitoring and management

Key Responsibilities:

  • Coordinate and deliver remote cardiac rehabilitation care.
  • Monitor members health including medication blood pressure and psychological wellbeing.
  • Collaborate with healthcare providers to ensure seamless care.
  • Support clinical policy development and quality improvement.
  • Assist members to navigate their health journey to reduce avoidable hospital admission
  • Ensure compliance with company policies and regulatory standards.

Skills And Experiences

About you

As the ideal candidate youll need to demonstrate the following skill and experience:

Mandatory:

  • Formal and current qualifications as a registered nurse with cardiac experience
  • Relevant work experience (minimum 5 years)
  • Able to work in self-directed and autonomous manner
  • Understanding of the current health industry environment both public and private
  • Demonstrated strong stakeholder management and interpersonal skills
  • High IT literacy
  • Formal qualifications in a relevant field
  • Experience in a care planning or coordination role

Highly desirable:

  • Experience in cardiology or cardiac rehabilitation (Preferred).
  • Experience in a care planning or coordination role
