Job Responsibilities
Under general supervision provide direct nursing care to clients exclusively enrolled in the Ryan White program which includes: screening patients for eligibility medical and non-medical case management including treatment adherence evaluation counseling and providing a broad range of services that link clients to sub-specialty services. Responsible for case management intake assessments and performing comprehensive assessment of health care needs.
Duties:- Fulfill nurse protocol training requirements in HIV STD Immunization and other relevant areas to function as an expanded role PHN.
- Provide direct nursing care including physical assessments education treatment and develop and document a plan of care consistent with the clients level of need.
- Screen and refer clients to other community service providers as needed. Assist with referrals and follow-up treatment planning as indicated.
- Provide sexual health promotion and prevention education including counseling clients and family regarding the importance of medication adherence for viral suppression.
- Participate in staff meetings and assist with developing and implementing quality improvement projects as needed.
- Maintain accurate medical records documenting is a must assist with data entry and quality assurance monitoring.
Minimum Qualifications
Associate degree in nursing from an accredited college or university AND one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification;
OR
One (1) year of experience at the lower level PH Registered Nurse 1 (HCP190) or position equivalent.Additional Information

All approved teleworking employees must reside within the state of Georgia and depending on the responsibilities of the position must live within reasonable driving distance of the agencys primary worksite and be able to report to the office when required.
Current State employees are subject to State Personnel Board rules regarding salary.
DPH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. DPH will contact educational institutions to verify degree diploma licensure etc.
