Job Description

This position will be for the SNF. MDS Coordinators work to improve patient care and help long-term care facilities maintain federal funding. To do that they complete several crucial tasks throughout the day. Common duties and responsibilities for an MDS Coordinator job description include:Completing and updating MDS forms on all patients in the care facilityPerforming clinical assessments to gather information on residentsDeveloping care plans for patients to coordinate available servicesSubmitting updates and paperwork to maintain complianceParticipating in the admission and discharge processesConducting and attending meetings with other care team membersInvestigating complaints or issues from residents and their familiesEducating patients and their families
