Job Description
Diversified Life Care Partners is seeking experienced and compassionate Care Managerslicensed RNs LPNs or MSWsto join our growing team. This part-time position offers a unique opportunity to use your skills and life experience to make a meaningful impact in the lives of older adults and their families.
As an Aging Life Care Manager youll work closely with clients and their loved ones to navigate aging medical and life transitions with clarity and support. This is rewarding flexible work with room to grow into a full-time role as caseload increases.
What Youll Do
Conduct in-home assessments to evaluate client needs risks and goals
Create and implement personalized care plans across medical social legal and financial domains
Advocate for the client in healthcare long-term care and legal settings
Coordinate services such as home care rehab transportation and specialty providers
Provide guidance to families during crises or care transitions
Document findings recommendations and progress in our secure platform
Communicate and collaborate with families providers attorneys and financial professionals
Qualifications
Active license as RN LPN or MSW
Minimum 10 years of relevant experience (geriatric care home health hospice case management social services etc.)
Deep understanding of aging issues chronic conditions and senior care systems
Strong communication organizational and problem-solving skills
Compassionate client-centered approach to care
Comfortable working independently in the field (must have reliable transportation)
Ability to travel to clients homes in Louisville Lexington and Southern Indiana
Proficient in basic documentation email communication and mobile scheduling tools
