Job Description:
To partner with the Nursing Resource Coordinator Care Transitions team Center for Patient Flow and all units of UAB Hospital to facilitate early discharges. Coordinates the patient care experience for patients who have been clinically cleared for discharge but are awaiting completion of discharge process and/or are in need of the coordination of other services including but not limited to transportation from UAB Hospital. To assume responsibility for certain technical aspects of patient care as directed by the RN/LPN for the hospital. To assist with activities of daily living (ADL) as needed. To obtain and record vitals weights inputs and outputs. To properly escalate any patient care need this is beyond the scope of the role. To successfully complete an in-house PCT training program. To demonstrate an awareness of patient and personal safety. To perform a variety of administrative tasks in support of patients while assigned to the Discharge Lounge including but not limited to coordinating with ambulance services Care Transition team and/or Food & Nutrition Services as needed to arrange for transportation medications and/or nutritional support as needed
