Position Summary:
We are currently seeking a dedicated and compassionate Hospice Aide (Certified Nursing Assistant - CNA) to join our interdisciplinary care team. The Hospice Aide will provide personal care services to patients in their homes or residential facilities offering comfort support and assistance with daily living tasks during their final stages of life.
Key Responsibilities:
- Provide personal care (bathing grooming oral hygiene toileting dressing mobility assistance) as directed in the plan of care
- Assist patients with ambulation transfers and exercises
- Observe and report changes in the patients condition to the supervising nurse
- Maintain accurate documentation of services provided
- Offer emotional support and a comforting presence to patients and families
- Work collaboratively with the hospice team including nurses social workers chaplains and physicians
- Follow infection control and safety procedures at all times
Essential Functions:
- Provide direct patient care to patients under direction of the RN and according to the Aide Plan of Care.
- Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
- Provide necessary skill to appropriately report changes and document pertinent information and care rendered to patients to ensure continuity of care.
- Practice accepted infection control principles.
- Promote positive supportive respectful communication to patient/family and other employees.
- Provide an environment which promotes respect for patients privacy and property.
- Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.
- Contribute to the management and efficient operation of the agency and demonstrate effective time management skills.
- Provide skills necessary to perform treatments and procedures according to agency policy.
Qualifications:
- High school diploma or GED
- Active CNA certification in the state of Texas
- Minimum of 1 year experience in hospice home health or a similar care setting (preferred)
- Valid drivers license and reliable transportation
- Strong interpersonal and communication skills
- Compassionate dependable and emotionally mature
- Must pass background check and drug screening
