You will be updated with latest job alerts via emailChristianaCares Integrated Behavioral Health department is looking to hire a Full Time Behavioral Health Consultant to support the Linden Hill Primary Care practice.
The Behavioral Health Consultant will contribute to the overall clinical operation of the behavioral health department and the daily care of the outpatient population in primary care. Will function as a member of the clinical team through collaboration providing individual psychotherapy consultation assessment treatment and behavioral health education and will provide collaboration with and a conduit to Behavioral Health and support an integrated model of health care.
Work Schedule:
Monday- Friday: 8am- 4:30pm.
Opportunity to work a hybrid schedule available!
Hybrid schedule include four (4) in-person days and one (1) remote day.
No weekends/ holidays
Key Responsibilities:
Demonstrates the knowledge skills and abilities necessary to provide psychological care appropriate for the Primary Care population. This requires the ability to conduct assessments individual psychotherapy obtain and interpret information to identify age and culturally specific patient needs and to provide the care needed.
Demonstrates the ability to function in a consult and liaison role for the Primary Care department meeting the acute needs of patients and providing outpatient psychotherapy as appropriate.
Coordinates transition of care to community setting as necessary; maintains knowledge of resources including community providers and support organizations appropriate for the patient population served
Develops group support and therapy activity as appropriate for the specific Primary Care population and as needed within the Christiana Care Health System
Maintains connection and consultation with the Department of Psychiatry and Behavioral Health
Identifies service gaps pertaining to patients psychosocial needs and plans programs that address the psychosocial needs of patients.
Implements programs effectively and evaluates effectiveness of programs making modifications as needed.

Qualifications:
Masters or doctors degree which qualifies in their state for licensure or certification is required.
Licensed Clinical Social Worker or LCSW LPC or LPCMH preferred.
Marriage and Family Therapists (MFTs) and Mental Health Counselors (MHCs) with at least 2 years or 3000 hours of postmasters degree clinically supervised experience required and must be licensed in order to bill independently.
Minimum 2-3 years of experience in mental health setting preferred.
