LOCATION:

This practice sees patients in skilled nursing facilities (SNFs) assisted living facilities (ALFs) independent living facilities (ILFs) and private homes. They are currently hiring in the Northern Kentucky area and will work with providers during the interview process to ensure a mutually-agreeable geographic territory in which all patients / buildings will be located.

This organization is dedicated to delivering convenient high-quality primary care to geriatric patients in the comfort of where they call home. They currently boast a team of more than 15 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a providers office. Their services include among others primary care chronic care psychiatric care post-acute care transitional care and remote patient monitoring.
POSITION DESCRIPTION:
The nurse practitioner will serve as the primary care provider for a dedicated panel of geriatric patients. All patients are located in facilities or private homes.
In SNF settings (with no travel during the day) providers see an average of 15 to 16 patients per day.
When traveling between ALFs ILFs and/or homes providers see an average of 9 to 12 patients per day (this is adjusted based on actual travel time and number of locations).
Specific duties of the role include but are not limited to:
- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions
- adjust treatment plans as necessary
- administer medications treatments and other interventions as prescribed
- perform minor procedures and wound care as required (and as settings permit)
- monitor vital signs and patient responses to treatment
- educate patients and families on health management disease prevention and treatments
- provide guidance on lifestyle modifications and self-care practices
- work with physicians nurses and other professionals to coordinate comprehensive care
- ensure compliance with all relevant healthcare regulations and standards
The practice uses the eClinicalWorks EHR.
On-call is phone triage only with very low volume. Providers rotate the on-call responsibility with a shift every 10 weeks or less.
REQUIREMENTS:
- Kentucky NP license
- DEA
- advanced practice experience preferred not required
- new grads with RN backgrounds in home health trauma or ICU are encouraged to apply
