Job Description

Title: District Nurse(RN LPN)

Reports to: Student Services Administrator

Work Year:210 days

1.0 FTE/8 hours per day Monday through Friday

Start Date: August 18 2025

  • Sherwood School District pays the 6% employee PERS contribution upon eligibility.

GENERAL DESCRIPTION OF THE POSITION:

The employee in this position develops and implements health management plans conducts student health assessments and collaborates with communicates with building staff families and medical professionals.

QUALIFICATIONS: Knowledge Skills Abilities Required:

  1. Bachelors degree (BSN) from four-year college or university
  2. Valid state issued LPN or RN qualifications and license
  3. Capacity to work and collaborate with others
  4. Ability and commitment to build strong relationships with district office and building staff members
  5. A willingness to serve and do what is necessary to support success of the assigned student(s)
  6. A strong work ethic
  7. Ability to make decisions independently and to use initiative and judgment in accomplishing tasks following overall policies and objectives
  8. State issued valid drivers license

ESSENTIAL FUNCTIONS include the following. Performs some or all of the following tasks:

  1. Tracheostomy diabetes feeding care experience strongly preferred.
  2. Develops and implements health care plans for students collaborating with families and medical professionals.
  3. Conducts student health assessments.
  4. Reviews health conditions in classrooms as needed assesses care needs and modifies health protocols or education plans. Consults with physicians and parents to obtain necessary information and convey findings.
  5. Delegates health-related procedures to staff. Supplies appropriate training and supervision in accordance with state standards (RN only)
  6. Monitor and assess care needs of assigned student(s).
  7. Consult with medical providers and school staff to communicate changes to health care needs.
  8. Perform direct nursing duties such as medication administration tube feeding seizure management medical monitoring etc.
  9. Communicates with parents and staff regarding health needs.
  10. Assists in collecting and tracking immunization records and meet state reporting guidelines.
  11. Participates in IEP/IFSP processes.
  12. Travels to multiple classroom sites. Make home visits as necessary.
  13. Provides consultation in educational settings regarding school health needs..
  14. Assist with duties related to the education or care of assigned student(s) as directed by administrator.
  15. Participate and collaborate as requested in meetings with school teams.
  16. Maintain regular and consistent attendance and punctuality.
  17. Employ effective communication and collaboration strategies with the school team.
  18. Follow and support Sherwood School District policies and procedures.
  19. Abide by FERPA and HIPAA regulations.
  20. Maintain professional and technical knowledge by participating in professional development opportunities.
  21. Perform other duties as may be needed.

MINIMUM QUALIFICATIONS:

  1. Bachelors degree (B.A.) from four-year college or university;
  2. To perform this job successfully an individual should have the ability to use or learn to use e-mail and word processing.
  3. Certificates as determined by the District including a Bachelor of Science Nursing degree.
  4. Ability to make decisions for the care of the student in collaboration with school professionals and to use judgment in accomplishing tasks following overall protocols policies and objectives
  5. Working knowledge of current Oregon laws policies administrative rules and model programs related to school nursing.
  6. Demonstrated ability to speak and write articulately and communicate clearly to compose and produce clear coherent professional documents and to deliver concise succinct professional presentations to various stakeholder groups.

EVALUATION:

Performance of this job will be evaluated annually in accordance with the provisions of the School Boards policy on evaluation of administrative personnel. For contracted employees a written summary of job performance will be provided annually to the relevant agency.

PHYSICAL REQUIREMENTS:

Ability to perform the requirements of this job description and the assigned duties with or without reasonable accommodation. Within a typical workday the employee may be required to perform any combination of the following:

  • In an eight-hour day the employee may:
    • Stand/walk 4-6 hours
    • Sit 4-7 hours
    • Use voice 6-7 hours
    • Utilize computer 2-4 hours
  • The employee may on a daily basis:
    • Bend kneel squat reach
    • Climb stairs
    • Use hands for repetitive motions including single grasping fine manipulation and pushing/pulling a maximum of 20 pounds
    • Drive between District facilities for meetings
  • The employee may occasionally:
    • Lift items weighing up to 50 lbs.

WORKING CONDITIONS:

The employee works with groups of staff small groups of students or on a one-on-one basis. The working conditions are that of a typical school building. Minimal travel between District buildings is required; occasional travel to area Districts or ESDs for meetings and conferences should be anticipated.
