Reporting to the Cancer Center Director
The Oncology Social Worker will provide assessment (distress screenings) counseling referral and additional planning services to adult groups of oncology patients treated at Holy Cross facilities while also providing supportive services to patients and their families.
OSW will manage Oncology Referrals and outpatient oncology patients. The OSW assists in providing guidance to patients to overcome barriers related to Oncology Care.
Responsibilities:
The Oncology Social Worker is focused on improving the quality of life for cancer patients and their families using community-based services.
Reviewed clinical assessments and determining appropriate levels of care determining if additional clinical services are needed for infants to elderly patient population.
Effectively handled member crisis calls by identifying risk ensuring safety and coordinating timely access to care.
What you will need:
State of Maryland LSWA LGSW LCSW or LCSW-C licensure (or license eligible).
Bachelors or Masters Degree in Social Work.
One year of social work experience in an acute-care setting preferred. Demonstrated ability to work with all age groups of patients with an emphasis on adult and geriatric patients. Able to communicate effectively.
