Some highlights of this opportunity include:
Participate in the decision-making process in relation to admission to hospice at home or in the inpatient setting
Participating as a member of the Interdisciplinary Team in the development of individualized care plans
Certify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice Benefit
Consult with attending physicians regarding pain and symptom management
Coordinate efforts with the attending physician to provide continuity of care
Develop and monitor the medical component of the hospice program
Participate in the development coordination and implementation of hospice services and policies
Participating in the development of staff and volunteer training
Conduct home visits (in person and virtual) as a consulting physician when appropriate
Act as a hospice liaison and advocate with the national state and local physicians healthcare professionals partners and community
Perform related administrative functions as required and/or assigned
Part time 8 hours per week
