Seeking Dementia Program APP
Situated at the edge of Glacier National Park Logan Health offers an opportunity like none other against the backdrop of one of the most beautiful places in

Seeking part-time Licensed APP to support the development and expansion of our Dementia Program
Minimum of 12 years APP experience preferred
Experience with geriatric or memory care populations strongly preferred
Outpatient-based care
No call responsibilities
Travel required to patient homes within the Flathead Valley as needed
Some telehealth services may be incorporated
Collaborative team approach working closely with a Nurse Navigator and Medical Assistant
