Job Description:

The DHHS Division of Public Health is seeking a Chronic Disease Health Educator to support the Chronic Disease Community Health Education programs such as Falls Prevention Health Coaches for Hypertension Control DPP and Living Well. Ultimately the team would like to build a sustainable community health education program across the state to help Nebraskans live better lives.

This position significantly impacts Nebraskans by providing chronic disease prevention and control programs tailored to community needs. It offers lifestyle interventions aimed at helping individuals avoid or manage diabetes and cardiovascular diseases.

The role involves distributing and presenting information on chronic diseases to promote healthy living statewide ensuring services reach the appropriate individuals at the right times and locations. Additionally it monitors these services to assess goal achievement and addresses social determinants of health within local communities making this position essential for fostering long-term health among Nebraskans.



  • Provide Heart Disease Diabetes Obesity Nutrition and Physical Activity prevention outreach education and intervention.
  • Be a community partner liaison between our federal funding sources and programs in the community support the outreach to new or repeat partners interested in implementing our programs facilitate and monitor partner touchpoints and support the outcomes of Diabetes prevention and Heart Disease management.
  • Coordinate the Heart Disease and Stroke Prevention and Management Nutrition and Physical Activity focused initiatives and Diabetes Prevention and Management Program throughout the state.
  • Support development of statewide lifestyle change programs including teaching leader trainings ensuring program fidelity and partner outreach and promotion.
  • Develop and manage grant and contract agreements; complete budget monitoring and planning; develop program policies procedures and evaluation; provide ongoing program quality assurance and monitoring; outreach and technical support to various entities.
  • Support the office through data management and reporting while ensuring compliance with state and federal regulations.
  • Assist with research and writing the required plans grants and proposals and maintain internal databases and resources.

Requirements / Qualifications:

Minimum Qualifications: Bachelors degree or equivalent coursework/training in community health education biological science adult/continuing education or related field AND experience in public/community health education activities with responsibility for: developing evaluating and leading health education program activities; assessing health needs; writing and designing educational and informational material training aids and presentations; preparing grant proposals; or planning and leading group sessions. Related experience may substitute for a Bachelors degree on a year-for-year basis.

Preferred Qualifications: Masters degree in public or community health healthcare nutrition or related field. Minimum of one year of experience working in public or community health healthcare or related field. Experience in community health education programs prevention of heart disease and stroke and Diabetes prevention and control; and connecting community programs with health systems to improve heart disease and stroke prevention care and management may substitute for a Masters degree on a year-for-year basis.



Knowledge Skills and Abilities:

  • Knowledge of the principles and practices of public health planning and implementation and the principles of grant writing reporting and grant administration.
  • Knowledge of the practices and techniques of developing partnerships and collaborations.
  • Ability to design educational and informational materials training aids and presentations and plan and lead group sessions.
  • Ability to interpret and apply federal grant requirements agency rules policies and procedures and prepare grant proposals.
  • Ability to maintain accurate and thorough records.
  • Ability to communicate effectively and demonstrate professionalism.
  • Ability to prioritize and manage time; problem solve and make decisions; and balance needs requirements and workload for multiple projects.
