***AmeriMed Infusion Pharmacy - Servicing West Chester Ohio areas ***

As a Care Transition Nurse (CTN)withAmeriMed Infusion Pharmacy you will play a key role in providing coordinated home health services for referred patients. Youll collaborate with healthcare professionals identify patient needs and facilitate seamless transitions to home health care.

  • Collaborate with hospitals nursing homes and home health personnel to ensure quality care.
  • Must meet or exceed established monthly admission target(s) as provided by their supervisor and determined by the branch monthly budget.
  • Coordinate patient referrals evaluate eligibility for services and assist in arranging home health care.
  • Analyze patient needs and educate caregivers about home health options Medicare/Medicaid guidelines and insurance benefits.
  • Develop relationships with healthcare providers to promote home health services and best practices.
  • Participate in provider education and support hospital staff as a home health resource.
  • Conduct trend analysis and identify opportunities for enhancing home health service outcomes.
  • Contribute to Quality Assurance Performance Improvement (QAPI) initiatives.

Qualifications

  • RN or LPN license in the state(s) served or a BSW/MSW from an accredited institution with relevant state licenses.
  • Minimum of (2) years of healthcare experience preferably in home health or sales/marketing.
  • Strong communication assessment and decision-making skills.
  • Ability to build relationships educate and promote services effectively.
  • Committed to professional development and improving own skills.
