Job Description
A respected healthcare organization is seeking a compassionate and dedicated Field Nurse (RN) to join its care team. This role is ideal for an experienced RN who values flexibility work-life balance and patient-centered care. The position combines home visits with in-office documentation offering a balanced and rewarding workweek.
Responsibilities Include:
Perform in-home initial assessments and reassessments for patients
Develop implement and revise care plans in collaboration with interdisciplinary teams
Provide training support and clinical guidance to Home Health Aides and Personal Care Assistants
Educate patients and families on care medications and condition management
Complete thorough and accurate documentation
Participate in care planning and clinical team meetings
Ensure patient needs are met with quality dignity and compliance
Ideal Qualifications:
Active New York State Registered Nurse (RN) license
At least 1 year of nursing experience (home care experience preferred)
Strong communication and clinical assessment skills
Patient-focused mindset and ability to work independently
Reliable transportation for travel between patient homes
Proficient in documentation and electronic medical records
