Physical Therapist Community Hospital of San Bernardino
Join a compassionate and skilled rehabilitation team where your expertise as a Per Diem Physical Therapist will help patients regain function independence and confidence. At Community Hospital of San Bernardino we are looking for a licensed professional to provide high-quality care across a range of medical conditions and patient populations.
As a valued member of our therapy team you will evaluate and treat patients using evidence-based practices craft individualized treatment plans and collaborate with interdisciplinary teams to deliver patient-centered care. This role allows you to make a meaningful difference while maintaining flexible scheduling.
Key Responsibilities Include:
- Perform comprehensive evaluations and establish personalized treatment goals
- Administer therapeutic interventions in compliance with physician orders and professional standards
- Educate patients and caregivers on home exercise programs and recovery expectations
- Document patient progress accurately and timely in accordance with hospital policies
- Participate in discharge planning and coordinate with care teams to ensure optimal outcomes
- Provide guidance and oversight to support personnel and therapy students
- Contribute to quality improvement efforts and department initiatives

Qualifications
- Current registration certification or license as a Registered Physical Therapist by the respective state of which one practices
- BLS - Basic Life Support certification is required.
- Bachelors (BA BS) degree in Physical Therapy is required.
