Your role and responsibilities
This is a key role in the organisation leading the social care assessment arm of the Home First Reablement Team. The purpose of this role is to directly lead manage provider practice guidance and manage allocation of work to a team of Independence and Wellbeing Practitioners (IWPs) across Home First West Suffolk. Which is currently rated Outstanding by CQC.
Working closely with the local Home First Registered Manager and Senior Management team to support the reablement of people referred to the service following a setback in their independence or discharge from hospital.
This will involve working with people being supported directly by the Home First operational arm as well as with those who are supported by specialised Reablement providers commissioned by Home First.

The team
You will be supported by the Deputy Head of Home First as well as Social Work senior management. You will lead a team of around 20 IWPs across the whole of West Suffolk supporting them carry out Care Act assessments on Reablement customers.
Home First prides itself on collaboration and teamwork so you will be part of the wider Home First Management Team which oversees a team of around 450 staff and supports several thousand people in the community every year. You will also have close links to the local Integrated Neighbourhood Teams you will work alongside colleagues from both Social Care and Local Health Teams.
The team has embraced hybrid working with the core base being Bury St Edmunds and smaller offices in 3 other locations across West Suffolk. As part of the management team it is important that you are visible and accessible to the team.
