As a 501(c )3 non-profit company NTSOC does not compromise care for the sake of profit. Our model is to care for clients first not fund a corporate owner. NTSOC serves southern Colorado families of children/young adults with disabilities homebound seniors and veterans by providing compassionate quality home care physical therapy CNA training and outpatient therapy. We pursue the best possible outcome as defined by the client family members and care team.
Salary: $72800 to $85000
5K sign on bonus paid out in the following increments:
$2500 after 3 months and the additional 2500 at 6 months
Benefits:
Provides comprehensive assessment of patients to determine eligibility and continued eligibility.
Reports significant findings to the appropriate physician.
Submits documentation in a timely complete and accurate manner.
Implements physicians orders in a timely complete and accurate manner.
Performs comprehensive coordination of care and case management for each assigned patient.
Plans coordinates and implements the patients plan of care in compliance with Agency and governmental regulations.
Performs patient care visits in accordance with Agency productivity requirements. Provides patient care delivery in a manner that enhances the overall mission vision philosophy goals and objectives of the Agency. Creates and maintains accurate field charts to facilitate continuity care.
Participates in a minimum of 75% of required meetings.
Acts as a consultant to other Agency staff.
Complies with all local state and federal laws.
Qualifications

Minimum Education: Graduate of an Accredited Nursing Program required.
Minimum Experience:1 year home health care experience preferred and 1-2 years of nursing experience in a health care setting required. Strong communication skills. Strong computer skills.
Licenses/Registrations: Colorado RN license in good standing with the Department of Regulatory Affairs. Up to date car insurance and driver license will be required.
Minimum Knowledge/Skills/Abilities:
Strong analytical/problem-solving skills to evaluate the quality-of-care services and resolve issues.
Strong time management and organization skills.
Strong computer skills. Ability to work with electronic medical records system.
Ability to comprehend analyze correct and formulate medical/nursing documents.
Ability to effectively communicate with staff clients and medical personnel.
Ability to advocate for and relate to clients/families and their concerns.
Ability to remain current in the skills and knowledge necessary to provide quality patient care.
Current knowledge of laws and regulations related to home care.
Ability to travel to client homes on a regular basis.
