As a Wellness Navigator with STAY Your Way you are the cornerstone of a members journey to lifelong independence and personalized well-being. Youll guide support and coordinate holistic wellness plans that allow members to remain in the comfort of their own homessafely confidently and with joy.
Key Responsibilities
Personalized Wellness Navigation
Manage a caseload of 3035 STAY Your Way members providing continuous coaching and proactive wellness planning.
Develop and implement tailored wellness strategies that support members physical emotional social and spiritual needs.
Conduct regular check-ins to monitor well-being identify risks and coordinate appropriate services or interventions.
Collaborate with the STAY leadership team to deploy care and services based on evolving member needs.
Design programs resources and activities that encourage aging-in-place and whole-person wellness.
Relationship Building & Member Advocacy
Build deep trusting relationships with members and serve as a consistent point of contact throughout their time in the program.
Act as an advocate for memberssupporting their goals coordinating care teams and helping them navigate changes or challenges in health.
Encourage healthy lifestyle habits and connect members to appropriate community and healthcare resources.
Foster partnerships with families healthcare providers and other care team members to ensure continuity and alignment of care.
Documentation & Communication
Maintain accurate and up-to-date documentation for each member including wellness goals service coordination and progress notes.
Ensure new member onboarding includes a comprehensive wellness overview to establish a strong foundation for support.
Uphold confidentiality and provide updates to the STAY Your Way team when members needs change or escalate.
Program Development & Professional Growth
Continuously explore and introduce new wellness initiatives resources and partnerships to enhance member experience.
Participate in ongoing training certifications and professional development in wellness aging and case management.
Collaborate with other Navigators and departments within Sunshine Meadows to share insights and improve program outcomes.
Qualifications
Qualifications
Background in health coaching case management gerontology social work or wellness-related fields.
Experience working with older adults and a strong understanding of aging-related needs and services.
Strong communication organization and advocacy skills.
CPR/AED certified or willing to obtain.
Emotionally mature empathetic and committed to the mission of supporting independence at home.
