Job Description
The Rockbridge Area Health Center is seeking a full-time Community Health Worker to join our team.
The Community Health Workers (CHW) will primarily be working with specific target populations community partners primary care teams and other agencies to improve access patient care and outcomes because of their deep understanding of the cultural norms of the people they serve as they are often members of the community themselves. CHWs find and meet people where they are in life and assist them in improving their life and health. CHWs focus on four essential areas: Relate Educate Navigate and Advocate.
- Education Credentials and Experience
- High school diploma or equivalent required.
- Any combination of 3 years health/social services experience and/or technical degree associate in arts degree or bachelors degree in a human service-related field preferred.
- CHW certification or willingness to obtain certification within 18 months of hire and maintain certification.
- Certified Application Counselor (CAC) certification or willingness to obtain certification within 18 months of hire and maintain certification.
- Current Basic Life Support (BLS) certification with hands on practicum required.
- Valid drivers license and reliable transportation required.
- Knowledge Skills and Abilities
- Must be able to always maintain confidentiality and exercise discretion.
- Individual must be professional self-motivated and have strong attention to detail communication teamwork customer service and computer skills.
- Must be able to maintain confidentiality and exercise discretion at all times.
