The Access Aged Care team provides co-managed primary care services to Aged Care residents across Australia.
Your New Role
Step into this exciting onsite full-time opportunity where youll report to the Nursing Regional Manager and become an integral part of our flourishing organisation. This newly created position is focused on improving the health outcomes of our valued customers.
In this role youll provide clinical management and coordination for our clients and partnering residential aged care facilities. Its perfect for a motivated and passionate individual who thrives in an autonomous role appreciating the freedom and flexibility offered by working from participating Aged Care facilities. Regular travel to partnering homes for clinical reviews will be required. While the position is advertised for Northern Beaches NSW occasional travel to other areas may be necessary.
As a Enrolled Nurse youll work as part of a multi-disciplinary team to ensure efficient and high-quality service delivery coordinating the clinical team with the support of the Nursing Regional Manager and administration team. Additionally youll contribute to the growth of the resident base through business development efforts. Proficiency in computer usage is essential and while prior telehealth experience is preferred its not mandatory.

Essential Criteria
- Diploma of Nursing.
- Experience in problem-solving and professional collaboration.
- Proficiency in IT and documenting customer needs.
- AHPRA Nursing Registration.
- Aged Care experience with leadership experience preferred.
- Current drivers license and reliable vehicle.
Skills & Experience
- Experience in a similar role ensuring individual health needs are met.
- Strong organisational and time management skills.
- Excellent relationship-building and communication skills.
- Proficiency in computer usage.
- Aged Care experience and understanding of clinical scope.
- Knowledge of person-centred care.
