The Role That Matters
As our In-Home CHSP Coordinator youll be more than just a coordinator youll be a life-changer advocate and trusted partner to our valued clients. This isnt your typical aged care role. Youll:
- Be the primary contact and champion for clients navigating their care journey
- Design personalised care plans that promote wellness independence and joy
- Build meaningful relationships that make a real difference in peoples lives
- Drive innovation in care delivery identifying new opportunities to enhance services
- Collaborate with our passionate team to deliver exceptional client-focused support
- Manage complex cases with creativity and compassion including clients transitioning to Home Care Packages and the new Support at Home program
What Youll Bring to Our Team

- Tech-Savvy: Well-developed computer skills to navigate modern care systems
- Clear Background: Current National Police Clearance (within three years)
- Work Authorisation: Australian citizenship or valid visa with working rights
- Health & Safety: Ability to meet all legislative and organisational requirements

- Industry Experience: Background in aged care community services or disability services
- Qualifications: Certificate IV in aged care/community services or relevant tertiary qualification
- Communication Excellence: Outstanding verbal written and interpersonal skills
