We are seeking a skilled and compassionate Part-Time/Per Diem Occupational Therapist (OT) to join our home health care team. In this role you will help patients regain their independence by improving their ability to perform daily living activities in the comfort of their homes. The ideal candidate will have a passion for helping individuals recover from illness injury or surgery while providing tailored care plans that meet each patients unique needs.
Key Responsibilities:
- Conduct thorough patient evaluations to assess physical cognitive and functional abilities.
- Develop and implement individualized treatment plans aimed at improving daily living skills fine motor skills and overall functional independence.
- Provide hands-on therapy including exercises and activities designed to improve strength coordination and mobility.
- Educate patients and their families on techniques and strategies to enhance independence and promote safety within the home.
- Collaborate with physicians nurses and other healthcare professionals to ensure coordinated patient care.
- Document patient progress treatments and changes in condition in the electronic medical record (EMR) system.
- Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
- Adhere to agency policies procedures and state/federal regulations to ensure high-quality care and compliance.
Qualifications:
- Active and unrestricted Occupational Therapist (OT) license in Florida.
- Minimum of 1 year of clinical experience preferably in home health rehabilitation or a related field.
- Strong understanding of home health care practices rehabilitation principles and state regulations.
- Proficiency in electronic medical record (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage multiple patients effectively.
- Compassionate patient-focused and detail-oriented.
- Valid drivers license and reliable transportation.
