JOB SUMMARY
The Population Health Associate functions to support population management activities. The Population Health Associate collaborates with Nurse Care Managers and the entire care team to help coordinate population health-related services with the goal of achieving positive health outcomes. Job responsibilities include but not limited to using reports to identify patients who need periodic follow-up gaps in care lapses in care and condition specific periodic testing/screening. These interactions will primarily be achieved through outreach via telephone e-mail or mail.JOB DETAILS AND REQUIREMENTS
Type: Part-time 48 hours per 2 week pay period benefit eligible
Typical hours: Day shift
Education:
- High School diploma or GED- required
Certification/Licensure:
- Certification as Medical Assistant required by a nationally recognized credentialing organization- preferred
Experience:
- A minimum of one (1) year of clinical experience in healthcare setting (hospital long-term physician or clinic practice or insurance)-required.
- Previous experience with Electronic Medical Records (EMR) as well as Microsoft Office (Word Excel Outlook)- required
