Job Description
Work Schedule:
50% FTE Day shifts Monday through Friday. The position is eligible for up to a $2000 sign-on bonus (pro-rated based on FTE). This is a hybrid role. When in person you will work at our Administrative Office Building in Madison WI.

We are seeking an Occupational Therapist (OT) to join our Home-Based Hospital Care Team! You will:
- Deliver acute hospital-level care to patients in their homes using an evidence-based and patient-centered therapy approach.
- Help patients regain independence improve daily function and safely navigate their home setting reducing hospital stays and improving outcomes.
- Work in collaboration with the interdisciplinary team to ensure that patients receive exemplary care in their home.

- Bachelors Degree in Occupational Therapy Required
- 2 years prior relevant experience Preferred
- Licensure as an Occupational Therapist in the State of Wisconsin Upon Hire Required
- Basic Life Support (BLS) certification within 3 months Required
