Job Description
We are seeking a caring and reliable Part-Time Licensed Practical Nurse (LPN) to join our home health care team. As an LPN you will provide compassionate patient-centered care to individuals in their homes under the supervision of a Registered Nurse (RN). You will play a critical role in helping patients manage their health recover from illness or surgery and improve their quality of life while staying in a familiar and comfortable environment.
Key Responsibilities:
- Deliver direct patient care including monitoring vital signs wound care and administering medications.
- Follow established care plans under the supervision of an RN or physician.
- Assist patients with daily living activities including mobility hygiene and nutrition.
- Educate patients and their families about home care practices medication administration and health management.
- Collaborate with healthcare providers and the home health team to ensure the best possible patient outcomes.
- Document patient progress health status and care provided in the electronic medical record (EMR) system.
- Monitor for any changes in a patients condition and promptly report to the RN or physician.
- Adhere to agency policies procedures and state/federal regulations to ensure compliance and quality care.
Qualifications:
- Active and unrestricted Licensed Practical Nurse (LPN) license in Florida.
- Minimum of 1 year of clinical experience preferably in home health or a similar setting.
- Strong knowledge of home health care practices and state regulations.
- Proficiency in electronic medical record (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently manage multiple patients and prioritize tasks effectively.
- Compassionate patient-focused and detail-oriented.
- Valid drivers license and reliable transportation.

Compensation: $45.00 - $55.00 per hour
