We are seeking an experienced and compassionate Part-Time/Per Diem Physical Therapist (PT) to join our home health care team. As a PT you will provide individualized physical therapy services to patients in their homes helping them regain mobility strength and independence after illness injury or surgery. You will collaborate with a multidisciplinary team to ensure that each patient receives personalized care and achieves their rehabilitation goals.
Key Responsibilities:
- Perform comprehensive patient evaluations to assess mobility strength balance and functional limitations.
- Develop and implement individualized treatment plans to improve physical function mobility and overall quality of life.
- Provide hands-on therapy including therapeutic exercises manual techniques and other interventions to restore movement and function.
- Educate patients and their families on safe mobility techniques home exercises and strategies to prevent injury or complications.
- Monitor and document patient progress adjusting treatment plans as necessary to meet therapy goals.
- Collaborate with physicians nurses and other healthcare professionals to ensure coordinated patient care.
- Maintain accurate and timely documentation of patient evaluations treatments and progress using the electronic medical record (EMR) system.
- Ensure compliance with agency policies procedures and state/federal regulations for home health care.
Qualifications:
- Active and unrestricted Physical Therapist (PT) license in State.
- Minimum of 1 year of clinical experience preferably in home health rehabilitation or a related field.
- Strong knowledge of physical therapy principles and home health care regulations.
- Proficiency in electronic medical record (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a diverse patient caseload effectively.
- Compassionate and patient-focused with a commitment to improving patient outcomes.
- Valid drivers license and reliable transportation.
