Your Role:
As a Senior Doc Provider youll care for seniors where they feel most comfortabletheir homes assisted living residences and care communities across Palm Desert Region. Youll be responsible for:
- Conducting patient visits with care efficiency and clinical excellence
- Managing chronic conditions and coordinating care across settings
- Collaborating with a supportive clinical and administrative team
- Providing consistent compassionate communication to patients and families
