Responsibilities:
- Conduct assessments and develop customized treatment plans for clients based on their individual needs and goals.
- Provide skilled physical therapy services or home health care services to clients in their homes.
- Monitor progress adjust treatment plans as needed and communicate effectively with clients families and healthcare team members.
- Educate clients and their families on therapeutic exercises home safety measures and techniques for promoting independence and well-being.
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for clients.
Requirements:
- Physical Therapist: Valid license to practice physical therapy in the state of Virginia.
- Previous experience in home health care or physical therapy preferred.
- Strong clinical assessment communication and interpersonal skills.
- Compassionate and client-centered approach to care.
- Age Friendly care
- CHAPS accreditation knowledge or experience is a plus.

Compensation: $80000.00 - $120000.00 per year
