The Opportunity:
Position Overview:
This position directs supervises coordinates evaluates and participates in Community Health Specialist also referred to as Community Health Worker (CHW) activities within the community health center for the purpose of developing standard approaches to enhanced patient care in the clinical setting. This position is responsible for planning implementing supervising staff developing policy recommendations and providing technical assistance for all related program activities. This position is also responsible for developing and maintaining community partnerships.
As the CHW Program Supervisor your duties will include:
Developing external relationships and strategic partnerships with healthcare and community organizations.
Representing the Community Health Center providing information and technical assistance as needed including responding to difficult problems and questions raised by partner agencies clients or staff.
Meeting with community partners to explain program goals requesting cooperation offering assistance or negotiating agreements.
Working with program data and outcomes including metrics development and documentation across a variety of systems including electronic health records or community information portals.
Developing implementing and maintaining plans for achieving program objectives and operational goals.
Developing/modifying procedures and guidelines to meet changing program conditions or goals in order to increase the effectiveness of operations.
Supervising Community Health Specialist staff including Community Health Workers and Outreach Workers. Duties Include: training and coaching staff evaluating performance recommending or initiating personnel actions interviewing and recommending or hiring new staff members; transfers or disciplinary action to insure adequate and competent staffing; assigning work developing work procedures consistent with county and department policy; and monitoring work performed by staff In order to meet agreed-upon goals objectives and target dates.
Collaborating with Primary Care and Dental Teams to develop and oversee specific strategies related to enhancing patient experience of care.
Developing new programs researching and developing related program materials.
Collecting and analyzing data and information concerning key program outcomes and policies.
Evaluating and developing processes and policies based on community health center patient care needs and compliance.

Minimum Qualifications/Transferable Skills*:
Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional educationtraining and experience will be evaluated. Candidates should submit a 1 page cover letter and (not more than) a 2-page resume which addresses the following minimum and preferred qualifications:
Equivalent to a bachelors degree and
Three (3) to six (6) years of experience that demonstrates the ability to perform the duties of the position which must include at least:
Minimum of three (3) years direct experience with primary care clinical services or community health services (this may include serving as a traditional health worker or community health worker)
Minimum of one (1) years of experience in program leadership or supervision equivalent roles
Preferred Qualifications/ Transferable Skills*:
You do not need to have the following preferred qualifications/ transferable skills to qualify. However keep in mind we may consider some or all of the following when identifying the most qualified candidates.
Certification as a Community Health Worker or Traditional Health Worker
Experience in developing and supporting care coordination case management and CareSTEPs practices within an Electronic Health Record such as Epic
Please clearly explain on your application how you meet any of the following preferred qualifications/transferable skills.
Professional experience related to addressing and implementing community based interventions policies or processes designed to improve health for primary care populations
Experience working in a clinical environment including any experience within a federally qualified health center (community health center)
Specific experience in developing or implementing actions or programs designed to improve health outcomes. This may include:
Evaluating community access to healthcare
Experience facilitating key improvement metrics
Developing trainings to improve team or staff knowledge
Collaboration on primary care clinical programs or patient populations
