The Opportunity:

Position Overview:

This position directs supervises coordinates evaluates and participates in Community Health Specialist also referred to as Community Health Worker (CHW) activities within the community health center for the purpose of developing standard approaches to enhanced patient care in the clinical setting. This position is responsible for planning implementing supervising staff developing policy recommendations and providing technical assistance for all related program activities. This position is also responsible for developing and maintaining community partnerships.


As the CHW Program Supervisor your duties will include:

  • Developing external relationships and strategic partnerships with healthcare and community organizations.

  • Representing the Community Health Center providing information and technical assistance as needed including responding to difficult problems and questions raised by partner agencies clients or staff.

  • Meeting with community partners to explain program goals requesting cooperation offering assistance or negotiating agreements.

  • Working with program data and outcomes including metrics development and documentation across a variety of systems including electronic health records or community information portals.

  • Developing implementing and maintaining plans for achieving program objectives and operational goals.

  • Developing/modifying procedures and guidelines to meet changing program conditions or goals in order to increase the effectiveness of operations.

  • Supervising Community Health Specialist staff including Community Health Workers and Outreach Workers. Duties Include: training and coaching staff evaluating performance recommending or initiating personnel actions interviewing and recommending or hiring new staff members; transfers or disciplinary action to insure adequate and competent staffing; assigning work developing work procedures consistent with county and department policy; and monitoring work performed by staff In order to meet agreed-upon goals objectives and target dates.

  • Collaborating with Primary Care and Dental Teams to develop and oversee specific strategies related to enhancing patient experience of care.

  • Developing new programs researching and developing related program materials.

  • Collecting and analyzing data and information concerning key program outcomes and policies.

  • Evaluating and developing processes and policies based on community health center patient care needs and compliance.
