Job Title: Social Worker Community Mental Health Team Rochdale
Location: Rochdale
Pay Rate: 30
Liquid Personnel is currently recruiting for a Social Worker to join a Community Mental Health Team based in Rochdale.
What will your responsibilities be
- Undertake statutory adult social care functions within a secondary mental health service working with individuals experiencing severe and enduring mental ill-health.
- Complete strengths-based assessments scheduled and unscheduled reviews care planning and commissioning of support in line with individual needs.
- Carry out Mental Capacity Assessments Section 42 safeguarding enquiries and facilitate timely hospital discharges in collaboration with health colleagues.
- Apply sound knowledge of key legislation including the Care Act 2014 Mental Capacity Act 2005 Mental Health Act 1983 and associated frameworks such as DoLS CHC and data protection regulations.
- Work in a co-located multi-disciplinary team setting across the Mental Health Review Team (Middleton) and CMHT West (Rochdale) supporting service users both within and outside of borough placements.

Qualifications and Experience:
- SWE registration
- Eligible to work in the UK
- Hold a full UK license
- Post-qualified experience
- Degree level or equivalent in Social Work
