Job Description

Optometrist

An Optometrist is needed in Hammond Indiana.

Known as the gateway to the Chicago metropolitan area Hammond combines the affordability of Northwest Indiana with easy access to city amenities Lake Michigan and forest preserves. This position is with a community-based FQHC dedicated to providing integrated health services to underserved and low-income populations across the region.

Salary and Benefits

  • Salary Range: $120000 $145000 depending on experience
  • Schedule: 3240 hours per week
  • Vacation: 120 hours per year
  • Personal Time: 40 hours per year
  • CME: 40 hours per year and $2500 allowance
  • Holidays: 10 paid holidays annually
  • Medical Dental and Vision Insurance
  • Life Insurance: 1.5x annual salary
  • Short-Term and Long-Term Disability: Covered
  • Retirement: 401(k) with 4% employer match
  • Malpractice Insurance: Covered through FTCA
  • Relocation Assistance: Available

Responsibilities:

  • Provide full-scope optometric care in an outpatient clinic
  • Diagnose and treat eye diseases such as glaucoma cataracts and macular degeneration
  • Prescribe corrective lenses and manage contact lens fittings
  • Refer patients to ophthalmology or specialty care when needed
  • Develop and follow departmental procedures and quality assurance standards
  • Participate in administrative meetings and care team collaborations
  • Maintain compliance with licensing and credentialing standards
  • Reports to: Director of Optometry

Requirements:

  • Graduate of an accredited Optometry program
  • Licensed Optometrist in the State of Indiana
  • Valid Indiana Optometric Legend Drug Certificate
  • Valid CPR Certification
  • 2 years of experience preferred
  • Strong interpersonal and problem-solving skills
  • Commitment to patient-centered and team-based care
