Job Description

The San Francisco Homeless Outreach Team (SFHOT) works collaboratively in small teams to engage and stabilize individuals experiencing homelessness to help establish treatment and to find permanent housing.

The Street Crisis Response Team Supervisor works closely with a team of other licensed clinicians to provide supervision and administrative coordination for a team of wellness specialists assigned to the Street Wellness Team.

Comprised of community paramedics and EMTs from the San Francisco Fire Department (SFFD) and outreach workers the Street Wellness team will respond to 911 and 311 service calls dispatched to focus on well-being checks and situations that require immediate attention but do not meet the threshold of an acute behavioral health crisis.

The team will perform medical behavioral and social needs assessments render immediate aid if needed and will be able to offer meaningful connections to services and housing. The team will be deployed in an SFFD vehicle and have the ability to provide transportation services to individuals who might need that as part of the engagement.

Under the supervision of the Program Manager and the Chief Program Officer the Street Crisis Response Team Supervisor provides leadership in overseeing client outcomes i.e. benefits health care permanent housing while providing quality assurance and operational oversight.

The Street Crisis Response Team Supervisor functions within a team composed of staffing from The Department of Homelessness and Supportive Housings (DHSH) and Heluna Health a private non-profit and interact with other City Departments non-profit and for-profit organizations. The Street Wellness Supervisors work occurs within the context of HSHs clinical policies including Harm Reduction and the Recovery Philosophy.

The work schedule is expected to be 8-hour shifts 5 days a week. The operations for this program are 7 days a week. Exact shift hours and days assigned are TBD.

ESSENTIAL FUNCTIONS

  • Works as part of a team to provides administrative oversite for Street Wellness workers
  • Responds to team requests for support especially around crisis intervention; provides effective de-escalation and crisis intervention including placement of 5150s evaluations for DTS/DTO/GD stabilizes clients who are homeless provides home visits in SROs street environments shelters treatment facilities and jail/prison as needed; works frequently in the public eye occasionally under media scrutiny.
  • Establishes positive relationships with clients staff and other providers in community; fosters cooperative work environment. Assists staff in preparing notes for individual cases which may include diagnostic assessment and treatment records.
  • Facilitates team meetings as appropriate typically weekly. Attends all other meetings as required.
  • Maintains records of all activities relating to homeless outreach prepares periodic reports as needed confers with supervisors and management team on formulation of policies and procedures. May also provide other duties as assigned.
  • Acts as On-Call Supervisor some night and weekend on-call shifts may be required.
  • Other duties as assigned.

MINIMUM QUALIFICATIONS

  1. Minimum Bachelors degree in social work or similar with two years of experience in outreach or similar with homeless people.
  2. Prior graduate level experience may be considered.
  3. Valid California drivers license and no history of major moving violations including DUIs during the past 5 years.
  4. Advanced level of knowledge of the client population and their complex needs including homelessness financial instability medical and mental health and substance abuse.
  5. Ability to work well with diverse staff and clientele including cultural language sexual identity gender and other diversity considerations within all neighborhoods in San Francisco.
  6. Ability to work successfully both independently and cooperatively.
  7. Ability to speak read and write clearly in English.
  8. Advanced level of competency using Internet email and Microsoft Word computer applications as well as ability and willingness to learn necessary programs needed for everyday job function
  9. Ability to use computers/internet to look up information and enter relevant notes.
  10. CPR and first aid certification within 90 days of hire.

PREFERRED QUALIFICATIONS

  1. Previous managerial experience a plus.
  2. Experience in a community-based setting serving ethnically diverse low-income clients. Knowledge of community resources and experience in community settings.
  3. Conflict resolution skills
  4. Customer service orientation.
  5. Analytical ability and skills to handle and solve complex organizational and clinical issues and problems.
  6. Computer knowledge/skills or ability to learn.
  7. Excellent oral and written communication skills
  8. Language capability: Spanish Cantonese

PHYSICAL DEMANDS

  1. Must be in good physical condition and capable of performing job duties requiring frequent use of the entire body including ability to stand walk climb stairs sit drive for extended periods of time exit and enter vehicles throughout your work shift.
  2. Ability to use a computer phone or office equipment for extended periods of time.
  3. Ability to successfully and efficiently complete tasks in an environment where background noise is present and interruptions may be constant.
  4. Must be able to lift a minimum of 35 lbs.
  5. With instruction the ability to assist persons with disabilities and to help transfer a 180 lb person and lift wheel chair in and out of a car or van.
