  • 27711 per annum (pro rata if part time)
  • 37 hours - Flexible working options available

Your role and responsibilities

  • undertake regular statutory visits
  • chair Pathway Review meetings to manage and assess risk or any unmet needs relating to housing; education/employment; finances health and emotional wellbeing
  • advice and guidance on all these subjects will help your clients form secure foundations in their local community and increase their chances of living independently.

You will need

  • a suitable practitioner level qualification at Level 3 or equivalent experience in relevant area of work
  • excellent communication skills with a commitment to listening to young adults around challenging topics such as family loss mental health difficulties substance misuse and criminality
  • a thorough understanding of safeguarding concerns and specific risks associated adult care leavers
  • good organisation skills and competency with computer systems; paperwork and case recordings.

The team

Our Leaving Care and Unaccompanied Asylum Seeking Children (UASC) teams are responsible for providing a leaving care service to care experienced young people aged 16 to 24. Team members work with young people to develop the skills and support they need to make a successful move away from care and towards more independent living.

The quality of the relationships created with young people is paramount as are working relationships which enable access to expertise and resources of a range of other services and agencies both within Children and Young People (CYP) and beyond.
