Job Description
Hospice Triage Nurse
Salary: $80-89k/yr BOE
Schedule: Friday 4:00 PM- Monday 8:00 AM
Location: REMOTE
Requirements of the Triage Nurse:
Licensed Registered Nurse in state of practice
BLS Certified
Excellence in communication and interpersonal skills to build rapport with patients and families.
Effective oral communication skills to interview patients and explain next steps as well as to pass information on to other members of the care team
Familiarity with medical records word processing and email software programs
About the Triage Nurse role:
Answer triage calls during assigned shifts while maintaining a calm reassuring presence during emotionally intense situations.
Prioritize and respond to urgent calls promptly.
Providing guidance on managing hospice-related symptoms like pain nausea and difficulty breathing recommending appropriate medication adjustments or interventions.
Collaborating with the interdisciplinary team including physicians case managers and other hospice staff to ensure seamless care coordination and follow-up.
Educating patients families and caregivers on the dying process end-of-life care and symptom management techniques.
Accurately documenting all patient interactions interventions and follow-up actions in the electronic medical record.
Updating patient health information systems with notes
Use clinical judgment to evaluate symptom severity and urgency and triage patient and caregiver calls to determine the appropriate level of care.
Provide emotional support and education to families and caregivers over the phone.
Assist caregivers with crisis management and problem-solving.
Accurately document all interactions assessments interventions and follow-up.
Ensure compliance with state federal and hospice agency regulations and protocols.
Any other tasks assigned.
