Provides comprehensive care to individuals and families by utilizing the Nurse Practitioner process and collaborating with physicians and other health care professionals. The Nurse Practitioner is responsible for performing face-to-face encounters required for eligibility of the Medicare Hospice Benefit after 6 months (180th day) of hospice care and each subsequent recertification. Reviews patients medical eligibility for hospice services by performing a nursing examination and documenting the same in the clinical record. The Hospice Nurse Practitioner communicates effectively with the Hospice Medical Director Hospice Director and the Hospice Interdisciplinary Team.

- National certification or eligible as a Family Nurse Practitioner
- Licensure or eligible for licensure as a Nurse Practitioner in Virginia
- RN licensure in the State of Virginia
- Approved or eligible for approval for prescription writing privileges in Virginia
- Prefer current BLS certification
