Job Description
Coverage area: Torrance CA and surrounding areas
Duration: 13 weeks
Shift: 08:00 AM 04:30 PM (5x8-hour shifts)
Pay Rate: $72/hr (W2) $77/hr (1099)
- Provide physical therapy evaluations and develop individualized treatment plans for patients in their homes.
- Deliver therapeutic interventions to improve mobility strength and function.
- Educate patients and caregivers on safe practices and home exercise programs.
- Document visits and progress using the EPIC electronic charting system.
- Collaborate with interdisciplinary team members to coordinate care and achieve patient outcomes.
- Adhere to agency protocols and state/federal regulations governing home health practice.
- Travel to patient homes within the assigned region using personal transportation.
- Total hip/knee replacement
- Post-op spinal surgery
- Parkinsons Disease
- Multiple Sclerosis

- Traumatic brain injury
- Cognitive disorders
- Orthopedic and spinal conditions
- Active California Physical Therapist License (or eligible for pending license)
- Minimum 1 year of skilled PT experience required; Home Health experience preferred
- BLS certification (AHA)
- Must have reliable personal transportation
- EPIC charting experience preferred; Oasis documentation experience is a plus
- Able to treat a broad range of conditions including orthopedic neurologic and post-operative rehabilitation
- Guaranteed Hours: Facility may cancel 1 shift per pay period
- Weekend Requirement: May be required
- Mileage Reimbursement: IRS standard rate
- Holiday Pay: 1.3x standard rate on observed holidays
- Dress Code: No required scrub color
- Charting System: EPIC
