Are you a licensed and compassionate Physical Therapist with a passion for delivering personalized care in a home health setting Join our dedicated team in Temple TX and bring your clinical expertise directly to patients where it matters most their homes. As a full-time Home Health Physical Therapist youll play a crucial role in helping patients regain independence and improve their quality of life following illness injury or surgery.
Key Responsibilities
Conduct comprehensive physical therapy assessments and interpret medical histories
Develop and implement individualized treatment plans based on patient evaluations
Administer therapeutic exercises and functional mobility training
Educate patients and caregivers on recovery prevention and wellness strategies
Monitor and document patient progress adapting plans as needed to achieve goals
Collaborate with interdisciplinary teams to deliver holistic coordinated care
Maintain accurate timely and compliant clinical documentation
Qualifications
Masters Degree in Physical Therapy (DPT preferred)
Minimum 1 year of physical therapy experience in a home health setting
Active Texas PT license
Current CPR certification
Strong interpersonal assessment and critical thinking skills
Reliable transportation and valid drivers license (required for patient visits)
