As an Advanced Practice Provider youll play an integral role on our team by providing high-quality cost-effective care to patients.
We are seeking a Nurse Practitioner or Physician Assistantto:
- Provide complex symptom management recommendations.
- Work in collaboration with the Palliative Care Medical Director attending medical team and Palliative Care Team members to deliver palliative care to patients in the acute care setting with life-limiting chronic diseases.
- Effectively communicate with patients and family by utilizing concepts of motivational interviewing.
- Perform assessment process to include patient history physical psychosocial and spiritual assessment.
- Provide support counseling and information to patients and families dealing with end-of-life issues or with issues related to life-threatening or serious and complex illness.

Education:
Minimum
If Nurse Practitioner:
Masters or Doctorates degree from accredited Nursing Program.
If Physician Assistant:
Bachelors or Masters degree from accredited PA Program.
Licenses and Certifications:
Minimum -
If Nurse Practitioner:Active RN and APNP license in State of Wisconsin ANDactive board certification as a NP in Family Pediatric Acute Care or Pediatric Primary Care.
If Physician Assistant:Active PA license in State of Wisconsin ANDactive PA board certification.
All Nurse Practitioner and Physician Assistant applicants must have:Active DEA license or ability to apply for such license prior to or upon hire ANDCPR certification and advanced life support certifications if relevant to practice area (ACLS PALS NRP).
