Site: Toronto General Hospital
Department: UHN Connected Care
Reports to: Sr. Director UHN Connected Care
Salary: Commensurate with experience and consistent with UHN Compensation Policy
Hours: 37.5 per week
Status: Permanent Full-Time (contingent on funding)

Connected Care is a strategic business unit at UHN focused on developing and delivering innovative models of care. Our work focuses on transformative care experiences for patients essential care partners and care provider teams. We break down barriers to change bringing together policy makers funders thought leaders clinicians and patients to the table to build solutions together.

Connected Care partners with a number of groups to spread and scale transformative change models including partnering with OpenLab to lead the NORC Innovation Centre (NIC). The NIC aims to transform the way health care is delivered and experienced by seniors living in the community. The Centres mission is to advance a 21st century model of integrated care in NORC buildings across Toronto and to develop health social and technology interventions that support healthy aging. The initial focus for the Project Director will be to lead the spread and scale of UHNs NORC model of care across Ontario.

Position Summary:

Reporting to the Sr. Director UHN Connected Care the Project Director will be responsible for leading the implementation and operations of system transformation initiatives. The successful candidate will be responsible for working with Ontario Health Teams delivery and care partners across the health system to deliver new care models to support Ontarians. 

Duties:

  • Leads the expansion of innovative and proven models of care working with stakeholder groups across Ontario  including planning facilitation managing communities of practice and monitoring for impact  
  • Works closely with internal and external stakeholders to design implement and evaluate local efforts including a strong focus on systems level change and providing backbone support including change and project management
  • Assumes overall end-to-end ownership of initiatives throughout the project life cycle and ensures milestone and on-budget completion i.e. oversee the expansion of the NORC model from start to finish
  • Develops and evolves strategic partnerships across sectors to deliver services and programming
  • Regularly interfaces with project stakeholders at various levels to build and maintain strong relationships
  • Develops implements manages and reviews short and long-term plans and assesses alignment with mission and goals of UHN
  • Regularly monitors stakeholder engagement and reports on implementation progress risks and issues to Sr. Director Connected Care on a regular basis and identifies strategies to minimize/ resolve
  • Maintains relevant budget and collaborates regularly with project and operations managers; takes remedial action to attain budget targets in consultation with relevant team members
  • Manages procurement of goods and services vendor contract relationships and delivery
  • Manages site and clinical team members including capacity building and coaching
  • Provides input and recommendations to support various teams and subject matter experts relevant to assigned initiatives

Qualifications :

  • Undergraduate Degree in Health Administration Business or related field required; Masters degree in Healthcare Administration Business Administration or related field is preferred
  • Minimum 6 -10 years Healthcare Project and Management experience
  • Project Management Professional (PMP) designation and Change Management Certification are assets
  • Clinical operations experience is required
  • Demonstrated track record of successfully delivering complex multi-stakeholder projects in a public sector environment
  • Experience leading health care implementation projects preferable in an integrated care setting
  • Demonstrated ability to utilize both project and change management methodologies effectively
  • Able to facilitate and communicate effectively with leaders from both internal and external healthcare organizations and project stakeholders
  • Highly skilled at anticipating and analyzing risks and issues exploring alternatives and developing appropriate recommendations with project team and subject matter experts
  • Ability to independently manage multiple projects and prioritize competing needs
  • Ability to perform in multiple roles and contribute to project operational or strategic initiatives

Experience:

  • Excellent interpersonal and stakeholder relationship skills
  • Strong tactical and critical / strategic thinking skills
  • Proven ability to adapt to a fast-paced dynamic environment
  • Ability to communicate in a clear concise manner both written and verbal
  • Excellent at team-building listening coaching group facilitation and conflict resolution skills
  • Highly competent computer skills including Microsoft Office Google suite Zoom and Outlook applications
  • Able to manage ambiguity and adapt to shifting contexts
  • Skilled at integrative thinking and familiar with processes of co-creation
  • Excellent communicator who is adept at unifying stakeholders (e.g. government health system leaders
  • frontline workers seniors caregivers designers researchers industry)
  • Passionate about social innovation and creating positive change
