About the unit
Our sub-acute floor is a 59 bed Medicare unit that has residents with multiple clinical issues including CHF dehydration cellulites diabetes DVT and stable cardiac related issues needing monitoring. Nurses work closely with a variety of health care professionals including doctors social work dieticians PT/OT hospice and spiritual support. Our sub-acute unit offers a well-rounded opportunity for learning professional growth and personal satisfaction.
Our Chronic Care floor is a 60 bed Medicare/Rehabilitation and long-term care unit and is a great place to learn basic med-surg I.V. therapy & wound treatments nursing skills. Nurses on this unit along with host of healthcare professionals work to provide and implement the best treatment plan for residents. This fast paced and busy unit provide a great atmosphere for PT/OT residents to be motivated and reach their goals.
Our Dementia Care floor is a 60 bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses. The dementia care is where we create a fullness of life through the joy of relationships the art of caring and the spirit of living. This unit also works closely with a multidisciplinary team meets and discusses the residents plan of care.

Ourfacilityhas 3 floors and 9 unit (3 units per floor) we are able to provide care for long term care residents and post-acute rehab floor has 59 beds with a focus on sub-acute rehabthesecond floor has 59 beds which focuses on long-term care and sub-acute rehab and our third floor is a secured dementia unit. Nurse-patient ratios vary depending ontheshift andthepopulation you will be serving. We care for dementia residents long term care and sub-acute rehab residents. With 179 beds we accept but not limited to patient specialties such as general rehab neuro rehab cardiac respiratory and cancer. We also have various programs for residents to participate in multiple goal is to create a Fullness of Life throughthejoy of relationshipstheart of caring andthespirit of living.
Scope of work
Licensed Practical Nurse (LPN)
Under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the health care team contributes to the assessment planning implementation and evaluation of patient care. Informs patients and families about treatments and medications being received equipment being utilized and other related activities as appropriate in coordination with the RN/Mid-level Provider/Physician. Documents care provided via computer/written patient record in a manner consistent with guidelines.
Qualifications
Required Graduate of an approved LPN program.
Experience in Skilled Nursing/Long Term Care. Preferred
LIC-License Practical Nursing (LPN) - STATEMI State of Michigan Upon Hire required
CRT-Basic Life Support (BLS) - AHA American Heart Association Upon Hire required
