The Department
The role will work closely with both the Interconnected Care Team broader Operational teams and will report professionally through to the Digital Health Division. The Digital Health Division is responsible for enabling clinical staff to deliver timely quality care to our patients and carers through innovative use of information technologies and through strategic management of information and data across the organisation.
Position Summary
Alfred Health is transforming approaches to healthcare delivery through our Interconnected Care Program. We aim to optimise patient care coordination improve patient access maximise resource utilisation and enhance the overall healthcare experience for both patients and employees through innovative technologies process enhancements and new care models.Were looking for a dynamic and passionate Interconnected Care Digital Health Lead to support the operationalisation of new interconnected care models.
The Digital Health Lead will progress the integration of digital health systems used across our healthcare facilities and in patient homes ensuring high-quality patient care through innovative and accessible digital solutions they will support the development of innovative care models escalation processes and patient safety protocols that ensure the delivery of high-quality patient care. The primary focus will be operationalising acute care pathways both within and outside the hospital setting including the implementation of virtual care digital solutions to facilitate efficient and effective healthcare delivery beyond traditional hospital settings.
