ENC Wound Ostomy HBO

Compensation range:

$41.45 - $55.96

Your rate of pay will be based on applicable experience

POSITION SUMMARY:

The Licensed Clinical Social Worker provides assessment and intervention addressing attitudinal behavioral psychosocial spiritual and social factors for patients receiving diabetes wound ostomy orthopedic gestational diabetes nutrition cardiac and pulmonary rehabilitation services. The LCSW is also available to other Enloe outpatient clinics or inpatient services as needed. The LCSW has responsibility for working as a member of the Enloe clinical inpatient and outpatient healthcare team(s) to assist with and coordinate the care and services for assigned patients for an episode of illness or treatment in collaboration with the patient family physician patient care team and payors. The LCSW utilizes advanced psychosocial skills to facilitate the coordination of care by using the principles of assessment planning intervention and evaluation. The LCSW promotes and evaluates the effective utilization of resources using current knowledge awareness of community services and assuming a leadership role with the patient care team to achieve optimal clinical and resource outcomes. The LCSW has the responsibility of working as a member of our care team to improve the quality of life for patients and their families facing problems associated with chronic and life-limiting illness. The LCSW may provide supervision to students and/or graduate undergraduate interns in the field of social work or health care and may lead support and networking groups.

The LCSW will support the use and completion of advanced directives and other advanced care planning tools. In addition this role provides strengths-based person and family centered comprehensive psychosocial assessments and psychosocial interventions with the highest integrity related to cultural appropriateness and developmental stage along the continuum of illness including: Supportive counseling PsychoeducationAnticipatory guidance Advocacy Coordination of social services Linkage to community resources and Non-pharmacological pain and symptom management.

The LCSW assists with Community Connections/Systems Improvement: Manages and enhances community partnerships participates in System Improvement and Quality Initiatives and collaborates with other services and participates in Enloe Health initiatives to improve and develop care for those with a serious illness requiring complex care.

EDUCATION / TRAINING / EXPERIENCE:

Desired:

- Experience in acute medical care outpatient/ambulatory clinic and/or case management

- Experience with crisis counseling and working with people from diverse backgrounds and complex medical issues

- Two years experience in medical social work or counseling in a medical setting

LICENSES / CERTIFICATIONS:

Minimum:

- Current Licensed Clinical Social Worker (LCSW)

SKILLS / KNOWLEDGE / ABILITIES:

Knowledge of issues at end of life coping with chronic illness ethnic cultural and economic diversity and interventions at different life cycle stages. Must possess knowledge about navigating the medical and social systems that frequently present barriers to patients. Critical thinking analytical skills and an ability to draw on the principles of self-determination while evaluating ethical dilemmas and value conflicts. Assessment planning intervention and evaluation of biopsychosocial and spiritual factors in all aspects of patient care including issues impacting compliance/self-care related to disease or illness. Strong crisis intervention skills. Maintains and regularly updates social work skills and knowledge. Must possess expertise in communication with a multi-disciplinary team and within multiple levels of the organization. Must always be able to maintain strict confidentiality. Ability to build positive working relationships with physicians and external agencies. Capable of using Microsoft Office Suite Epic MIDAS and Chart Maxx. Must be able to fulfill the essential functions of the position.
