KEY ACCOUNTABILITIES:
Performs delegated patient care activities using evidence-based practice and specific competencies for the patient population served.
Checks patients vital signs height and weight as needed based on patient type.
Provides compassionate care and listens to patients concerns and guides them by answering any questions or concerns.
Identifies and escalates changes in patient status or patient risks to prevent crisis.
Collects clinical data and information on patients and reports to care team.
Repositions transfers ambulates and transports patients.
Assists patients with basic needs (such as hygiene eating using the restroom and dressing).
Obtains and reports patient intake and output.
Performs electrocardiograms (ECGs) as directed.
Performs phlebotomy and specimen collection.
Provides care that is age appropriate culturally specific and developmentally appropriate in an environment that promotes patient safety.
Demonstrates professional and therapeutic relationships with patients families team members and providers.
QUALIFICATIONS:
High School diploma or GED preferred Must be 18 years of age required expect for Peabody Manor The Heritage or Juliette Manor
Previous healthcare experience preferred
American Heart Association HealthCare Provider Life Support (BLS)
PHYSICAL DEMANDS:
Ability to move freely (standing stooping walking bending pushing and pulling) and lift up to a maximum of fifty (50) pounds without assistance
Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
Manual dexterity and hand-eye coordination to perform patient care procedures
WORK ENVIRONMENT:
Frequent exposure to sharp objects and instruments
Occasional exposure to moving mechanical parts fumes or airborne particles toxic or caustic chemicals and risk of electrical shock
Occasional high noise level in work environment
Standing and/or walking for extended periods of time
Transporting transferring positioning patients and/or equipment from one location to another; little likelihood for injury if proper body mechanics and procedures are followed
Possible exposure to communicable diseases hazardous materials and pharmacological agents
Occasional contact with aggressive and or combative patients
Position requires compliance with department specific competencies.
SCHEDULE:
Rotating 8 hour Days and PMs: 5:30a-2p or 1:30p-10p (Nights if needed)
Every other weekend required
Rotating holiday schedule required
UNIT DETAILS (Rehab-Neenah Hospital):
The Rehab unit is a 12 bed acute inpatient unit. Our average census is 7. Our most common diagnosis seen are stroke spinal cord injury traumatic brain injury and debility patients. Our average stay length of stay is 12 days. Our team is very collaborative between nursing therapy care management and providers who work towards progression of care and promotion of patient independence prior to the transition to the next level of care. Our Neenah hospital is a level II trauma center with a higher acuity patient population. Core therapy team members include a physiatrist two physical therapists two occupational therapists two speech therapists and one recreational therapist. Our team is dedicated to the highest quality of care and fosters great learning opportunities.
