QIDP - ICF/ID
Hills & Dales is a mission-driven nonprofit organization committed to building meaningful lives for individuals with intellectual and developmental disabilities including those with complex behavioral needs. We are seeking a compassionate and organized QIDP (Qualified Intellectual Disabilities Professional) to join our team and coordinate individualized care within our Intermediate Care Facility (ICF/ID) in Dubuque IA. This is a key leadership role that directly impacts the lives of our residents by ensuring quality person-centered services.
Schedule: Monday - Friday 8:00 AM - 5:00 PM primarily. **Some early mornings / evenings and weekends required.**
Oversee the active treatment programs for assigned residents in compliance with federal state and local ICF/ID regulations and CARF standards.
Facilitate individual program planning (IPP) meetings quarterly reviews and progress notes.
Lead and supervise Personal Assistants fostering teamwork and professional growth.
Coordinate new resident admissions discharges and pre-admission assessments.
Serve as a liaison with families guardians and interdisciplinary teams.
Monitor quality indicators compliance standards and staff performance.
Support and collaborate with department leadership on strategic and operational goals.
Provide emergency response coverage in the absence of the Facility & Support Director.
Maintain accurate documentation and uphold confidentiality standards.
Bachelors degree in social work human services or a related field as defined by ICF/ID regulations (required).
At least one year of experience working directly with individuals with intellectual and/or developmental disabilities.
Supervisory experience in a care-based or residential setting strongly preferred.
Strong organizational interpersonal and leadership skills.
Passion for advocacy and improving the quality of life for people with disabilities.
Must be free from prior convictions or employment history related to abuse or neglect.
Valid drivers license and insurable driving record required.
