About the Role
Our Home Allied Health team are seeking an enthusiastic Grade 2 Occupational Therapist to join the team. You will provide Occupational therapy services to clients under the Commonwealth Home Support Program (CHSP). This role involves working both autonomously and collaboratively within a multidisciplinary team to achieve timely and effective outcomes for clients. You will be required to provide high quality evidence based practice to support older adults to maintain their independence safety and well being in the community. .
Classifications for this position will range from classification range VF6-VF9 ($90235.60 - $103724.40 per annum ).
Your Contribution
AHPRA registered Occupational Therapist and Current Victorian Drivers License.
Well developed communications skills with focus on providing timely and effective clinical management and client goal setting.
Experience and excellent working knowledge of community based services & keen interest in care of over 65 year old clients.

About the Home Based Allied Health Service
The Home Based Allied Health team is jointly funded by the Victorian Home and Community Care Program for Younger People (HACC-PYP) and the Commonwealth Home Support Program (CHSP) and focuses on restoring or maintaining peoples independence improving functioning and wellbeing to support them to remain living at team forms part of the St Vincents Virtual & Home that has admitted services across Acute and Subacute streamed Units including Hospital in the Home Transition Care Bed Based. Non admitted services including District Nursing Residential in Reach Palliative Transition Care Program Home Based Allied Health.
Our team includes: Physiotherapy Occupational Therapy Speech Pathology Podiatry Dietetics and an Allied Health Assistant. The Home Based Allied Health Services are based at St Georges Health Service (SGHS) and the position requires staff to undertake home visiting duties.

Position Duties
- Provide high quality and evidence-based patient centered care.
- Demonstrate advanced clinical knowledge in the area of aged care and care of the older person
- Provide appropriate timely and effective clinical management to all patients through assessment treatment evaluation team liaison and discharge planning
- Complete client goal setting in consultation with patient/family/carers as appropriate. Goals are to be functional and measurable.
- Work in collaboration with others by communicating client assessment and treatment outcomes effectively with other team members and contribute to multidisciplinary team meetings.
- Demonstrate high level decision making skills to assess and reduce risks to complex clients by working autonomously and with the support of supervisors to address specific issues
- Undertake discipline specific risk assessment set priorities and work in a timely and efficient way according to program requirements.
- Assess for and arrange for supply of equipment to clients to improve safety maximise functional ability mobility and independence
- Provide written reports to relevant external organisations as required
- Active participation in therapeutic group work as required.
- Ensure all documentation client records statistics and activity recording are completed and managed in a timely manner consistent with SVHM SVVH and Government policies and procedures legislative and regulatory requirements.
- Manage workload with a high level of autonomy and responsiveness to program and organisational drivers.
- Actively participate in supervision professional development and performance review
- Contribute to the skill development of the Allied Health Assistants.
- Participate in quality and continuous improvement initiatives team processes and service development activities
- Complete annual clinical competencies required learning and accreditation in accordance with professional and legislative requirements.
Selection Criteria.
- Full and current registered as Occupational Therapist with AHPRA
- Minimum of 5 years experience in a health care setting with experience working with older clients in Community settings
- Pre-employment Victoria Police Safety Check
- Victorian Drivers License
