About the Role:
This position primarily focuses on outpatient and home-based palliative care with occasional inpatient consults to ensure seamless continuous support. Youll work alongside a collaborative team of physicians and advanced practice providers committed to compassionate patient-centered care.

- Provide expert palliative and hospice care across home clinic and community settings
- Collaborate closely with a dedicated multidisciplinary team
- Participate in call and weekend rotations to support patient needs
- Offer continuity comfort and guidance to patients and their families

- Board certified or eligible in Hospice & Palliative Medicine

Compensation:
The starting salary for this position is: $240000.00 (annually).
The actual compensation package could vary based on factors such as but not limited to the applicants experience internal equity and alignment with market data.
