The Clinical Dietitian is an integral member of the interdisciplinary health professional team and is responsible for the nutrition care disease management and education of patients. The Dietitian initiates a cost effective nutrition therapy comprised of patient assessment the development and implementation of a patient-centered goal-focused care plan with ongoing evaluation. Nutrition care plans are outcome-focused incorporate evidence based best practice are formulated to optimize both clinical and fiscal outcomes and emphasize health promotion and disease prevention. The Clinical Dietitian is responsible for the continuum of care of patients with effective evidence based strategies to self manage disease thereby decreasing the risk associated complications and hospital admissions. As part of patient care the Clinical Dietitian is responsible for educating patients/caregivers about their overall disease and especially how it relates to nutritional management. The Clinical Dietitian is part of and promotes a health care team approach to patient care.
The Dietitian will provide health assessment interventions and appropriate follow up in accordance with acts regulations policies standards guidelines mission and objectives of the Registered Dietitians of Canada the Nova Scotia Dietetic Association the Department of Health and Wellness - Province of Nova Scotia and the Nova Scotia Health.
The Highland Health Home and Learning Centre will be a learning and training environment where all NSH staff and physicians are expected to mentor and preceptor students and learners as part of their regular role.
Please note this position requires travel to facilities within Pictou County. A valid drivers license and access to transportation is required.
